What the DUTCH Test Actually Measures

DUTCH stands for Dried Urine Test for Comprehensive Hormones. It is the depth of the report that makes it useful, because it covers how your hormones move and how you process them, not only how much of each you have.

Estrogen, and how you clear it

not only how much estrogen you are making, but which pathway your body sends it down afterwards. Two women with identical estrogen levels can feel completely different depending on how they process it, and that difference changes what actually helps.

Progesterone

measured through its metabolites, which is where the picture behind PMS, spotting, short luteal phases, anxiety before your period, and disrupted sleep usually sits.

Androgens, including DHT

DHEA, testosterone, and the more potent androgens your body converts them into. This is the section that explains jawline acne, hair thinning, unwanted hair growth, and much of the PCOS picture.

Your cortisol pattern across a full day

four timed samples rather than one, showing whether your cortisol rises properly in the morning and falls at night. It also separates free cortisol from total production, which tells us whether you are making too little or clearing it too fast.

Why Not Just a Blood Test?

Blood is the right tool for plenty of things, and I order it constantly. These are the four questions it cannot answer, and they are the ones behind most of what women are told is normal.

cortisol is supposed to change across the day. A single blood draw at 9am cannot tell you whether your rhythm is intact, and the rhythm is usually the thing that has gone wrong.

how you process and clear a hormone matters as much as how much of it you make. Standard panels report the level and stop there, which is why two women with the same result can need completely different support.

Is the DUTCH test better than a blood test for hormones?

Neither is universally better, and they answer different questions. Blood is the right tool for thyroid, iron, blood sugar, and several other markers. The DUTCH test is stronger for cortisol rhythm across a day and for hormone metabolites, which is where symptoms often originate. In practice the two are frequently used alongside each other.

When in my cycle should I collect the samples?

For women who are still cycling, collection is usually timed to the luteal phase, roughly five to seven days after ovulation, so progesterone is measured when it should be at its highest. If your cycles are irregular or absent, the timing is set differently, which is one reason the test is planned in an appointment rather than simply ordered.

Can I do the DUTCH test while on hormonal birth control?

It can be done, but it will not show your own hormone production, because hormonal contraception suppresses it. The cortisol, melatonin, and nutrient sections still return useful information. If the question is what your own cycle is doing, that testing is usually better placed after you come off.

What happens after the results come back?

You get a full appointment going through the report in plain language, and a treatment plan built from what it found, using nutrition, targeted supplementation, and lifestyle support. Testing that is not followed by a plan and by follow-up is just information, which is not what you paid for.
